When your hospital treatment is complete, and after a period of recuperation, you should be able to return home. For many people, aftercare such as taking medication or visiting an outpatient clinic is essential after a stay in hospital.

The hospital staff should be keen to involve you in the decisions made regarding your discharge from hospital. You will be given the chance to discuss your needs and how they will be met. Discharge from hospital is a process that involves cooperation between patient, hospital staff, local authorities, primary care providers such as your GP, independent and voluntary sectors. The aim of the process is to ensure that you leave hospital when, and as soon as, you are ready to do so, and that you have everything you need for a full recovery when you return home.
