Under the data protection act 1998 you have the right to see or obtain a copy of your health records in manual or computer form. بموجب قانون حمايه البيانات 1998 لك الحق في ان ترى او ان تحصل على نسخة من السجلات الصحية الخاصة بك في شكل كتيب او الحاسوب. However, access can be refused under particular special circumstances, for example: ومع ذلك ، يمكن رفض الوصول لا سيما في ظل ظروف خاصة ، على سبيل المثال :

• when the healthcare professional believes that the information would cause you serious physical or mental harm, عندما الرعايه الصحية المهنيه ويعتقد ان المعلومات الخطيره من شأنه ان يسبب لكم الضرر البدني أو العقلي ،
• or access would make known information, which relates to a third person without their permission. او من شأنه ان يجعل الوصول الى المعلومات المعروفة ، والتي تتعلق شخص ثالث دون اذن منهم.

An application for access to health records, or to any part of a health record, may be made to the holder of the record by the following: تقديم طلب للحصول على السجلات الصحية ، او على اي جزء من السجلات الصحية ، ويجوز لصاحب سجل من خلال ما يلي :

• you, انت ،
• a person authorised in writing by you on your behalf, اي شخص مفوض كتابه من قبل لكم نيابة عنكم ،
• a parent or guardian of a child under 16 if this is in the child's best interests and not contrary to a competent child's wishes, احد الوالدين او الوصي للطفل دون 16 سنة اذا كان ذلك في مصلحة الطفل والذي لا يتعارض مع مختصه رغبات الطفل ،
• any person appointed by a court to manage the affairs of those incapable of managing their own affairs, اي الشخص المعين من قبل محكمة لادارة شؤون تلك غير قادرة على إدارة شؤونهم بأنفسهم ،
• or the personal representative of someone who has died, unless the deceased left instructions denying access to that person. او الممثل الشخصي للمن شخص قد مات ، ما لم يترك المتوفى تعليمات حرمها من الوصول الى ذلك الشخص.
